Thank you for hosting a SAVORlife Dinner!

As a host, I understand:

1. I determine the date, time, menu, and guest list.
    
2. Aunt Rita's Foundation is not responsible for providing food and beverages.
    
3. Aunt Rita's Foundation is not liable for any occurrence at my dinner.
    
4. My guests will contribute at least $35 by credit or debit card online or by check or cash at my dinner in order to be placed on the Fast Pass list for the SAVORlife Soiree.
    
5. I will provide Aunt Rita's Foundation with a completed guest list, so that guests with completed information can receive Fast Pass for the SAVORlife Soiree.
    
6. I will return the guest list and all checks to Aunt Rita's Foundation, 2700 N. 3rd Street, Suite 2012 Phoenix, AZ 85004 or I will give the guest list, checks and/or cash to the appropriate Aunt Rita's Foundation representative.
