Thank you for hosting a SAVORlife Dinner!

As a dinner host, I understand:

1. I determine the date, time, menu, and guest list.
    
2. Aunt Rita’s Foundation is not responsible for providing food and beverages.
    
3. Aunt Rita’s Foundation is not liable for any occurrence at my dinner.
    
4. My guests will contribute at least $25 by credit or debit card online or by check or cash at my dinner in order to be placed on the guest list for the SAVORlife Soiree.
    
5. I will provide Aunt Rita’s Foundation with a completed guest list.
    
6. I will return the guest list and all checks to Aunt Rita’s Foundation P.O. Box or I will give the guest list, checks and/or cash to the appropriate Aunt Rita’s Foundation representative.
